Today differential diagnosis between MS and other autoimmune disorders of the central nervous system is becoming more and more relevant, as new therapies are emerging. Biomarkers suitable to precisely identify MS patients are not yet available and the risk of misdiagnosis is high. Therefore, in those cases in which there is unexpected reactivation during treatment, the diagnosis should be redefined, as in the case presented here.
